So, LinkedIn makes it super easy now. You can add a "career break" and even specify that the break was related to caregiving responsibilities (isn't that great?!)

For your resume, I'd add a similar title/description that the one offered on LinkedIn - especially if you want to be open about it! If not, I would just add "career break" and have your LinkedIn profile and the information you'll share during the interview fill-in the blacks.

Unfortunately, with career breaks, there's no one/best way to present this information because (and I hate to even have to say this) the recruiters'/HR person's biases, stereotypes, and prejudices will have a lot of influence on whether you get call backs :(
